HIV/AIDS感染风险
偶遇的性伴可能在不知不觉中被感染上HIV/AIDS。所以，与偶遇性伴的未防护的肛交会冒传染的高风险，尤其对于“接受体液的”性伴一方来说风险更大。

考虑到当前HIV/AIDS的流行和世界范围其它性传播疾病(STDs)增加的因素，男女都应该要了解与新近偶遇认识的和不熟识的性伴经由肛交的感染风险。甚至熟识的性伴，如果他们依次有偶遇的性伴，那么他们也许不知不觉就已经感染了性传播疾病或艾滋病。对于肛门被插入、因而接受可能受到感染的精液的女子，这种感染的风险最大。进行插入的男子所冒风险较低，但也建议他们使用安全套。同性肛交是HIV/AIDS传播的可能途径。
最后，我们应该提及还存在着另外的有风险的行为：舔吮肛门(anilinctus，即舔肛、吻肛)。这种行为冒感染许多种严重的肠源性疾病的风险
(志贺氏细菌性痢疾、阿米巴痢疾、贾弟虫病、沙门氏菌病、寄生性蠕虫感染，等等。)以及冒感染像淋病、梅毒和乙型感染等“传统的”性传播疾病的风险[1]。

Casual partners may be infected with HIV/AIDS without knowing it. Therefore, unprotected anal intercourse with them carries a high risk of infection, especially for the “receiving” partner.

In view of the current HIV/AIDS pandemic and the world-wide increase of other
sexually transmitted diseases (STDs), males should understand the risk of infection through anal intercourse with casual, new, and unfamiliar partners. Even familiar partners may unknowingly have become infected if they, in turn, had casual partners. The risk is greatest for the partner being penetrated and thus receiving possibly infected semen. The penetrating partner runs a lower risk, but is also well advised to use a condom. Homosexual anal intercourse is one of the main paths of HIV/AIDS transmission. Finally, there is another risk behavior that should be mentioned: Licking the anus (anilinctus). This carries the risk of infection with a number of serious intestinal diseases (shigellosis, amebiasis, lambliasis, salmonellosis, parasitic worm infestations, etc.) as well as the “classic” sexually transmitted diseases like gonorrhea, syphilis, hepatitis B, etc.
