What are the questions?
A sex history has to cover many areas in order to be useful to both therapist and client.
At the very least, it should include detailed information about the following items:
Personal background
(sex, age, level of education, employment, relationship with parents, siblings and other relatives, friends, marital status, parenthood, religious commitment, moral values, interests, hobbies, sports activities, illnesses or infirmities, medications taken, alcohol and nicotine consumption etc.)
Sex education
(which sexual information was received when from whom and under which circumstances?)
Masturbation, Fantasy and Dreams
(when what and how often?)
Sexual experiences with others
(when what with whom and how often? Heterosexual and homosexual experiences? Sex with close relatives?)
Use of contraception
(if at all and what when?)
Marital history for each marriage
(sexual satisfaction in marriage)
Current partner
(since when? Plans for the future? Sexual satisfaction in current relationship)
Sex outside marriage
(with whom, when and how often?)
Group sex activity
(when and how often under what circumstances?)
Prostitution
(paying or being paid? when, where, why, and how often?)
Erotic responses
(how? to what? when?)
Sexual violence
(victim or/and perpetrator? How often? When, where, who, under which circumstances?
Sexual problems and dysfunctions
(Which when where with whom under which circumstances?)
